Question:

We have a patient with a previously placed implantable cardioverter-defibrillator (ICD). We took out the existing generator, inserted a new generator, capped the old right ventricular (RV) lead, and added a new RV lead. The old atrial lead was attached to the new generator. How would we code this?

Answer:

You would assign the following codes:

33241 Removal of pacing cardioverter-defibrillator pulse generator only
33249 Insertion or replacement of permanent pacing cardioverter-defibrillator system with transvenous lead(s), single or dual chamber for this case.

Since the old RV lead was capped, not removed, you would not assign the following code for a lead extraction or removal:

33244 Removal of single or dual chamber pacing cardioverter-defibrillator electrode(s); by transvenous extraction
